AED Drones Beat Firefighters to Cardiac Arrest by 5 Min

James City County, Virginia, has become the first locality in the state — and only the second in the US — to test drone-delivered automated external defibrillators (AEDs) for real 911 cardiac arrest calls. In one test flight, the county's Air10 drone reached the scene in 2 minutes 30 seconds, more than 5 minutes ahead of the fire department's 8-minute-6-second average response time. A pilot phase with live dispatches begins August 1, 2026.


Background

Sudden cardiac arrest kills roughly 350,000 people outside hospitals every year in the US, and survival odds drop 7% to 10% with every minute defibrillation is delayed, according to the American Heart Association, which is funding the James City County research. In many suburban and rural areas, fire and EMS crews can take 8 minutes or longer to arrive — well past the window in which an AED does the most good.

AED (Automated External Defibrillator) is a portable device that analyzes a heart's rhythm and delivers a shock to restore normal beating; modern units talk bystanders through every step, so no medical training is required to use one. Sweden pioneered AED drone delivery in the 2010s and recorded the technique's first confirmed lifesaving use in 2023, a precedent PickDrones covered in its overview of drones in medicine.


How the James City County Program Works

The James City County Police Department (JCCPD) began weekly test flights in April 2025 under a "Drone as First Responder" (DFR) program built with Duke University and Virginia Commonwealth University (VCU) researchers, in partnership with the county's Fire Department and the Peninsula Regional Emergency Communications Center. Nine officers from the department's Specialized Response Team, formed in 2021, fly the missions.

When a 911 call reports a possible cardiac arrest, an Air10 drone launches and flies to the location, then lowers the AED to the ground on a tether for bystanders to retrieve. The device gives audible step-by-step instructions and stays in contact with dispatchers until paramedics take over. The Federal Aviation Administration (FAA) issued the waivers needed for the flights, which is why the county could only test the concept incident-by-incident rather than fly it as routine airspace operations.

Testing progressed through three phases, starting with a 2-mile radius around the county's Law Enforcement Center. The pilot phase that starts August 1 will limit dispatches to a 5-mile radius around Fire Station 1 at 3135 Forge Road in Toano, during limited hours, while the county and researchers measure real-world reliability before expanding range, hours, and drone count.


Early Results and Official Reaction

The most-cited test flight — an AED delivered in 2 minutes 30 seconds versus a fire crew's 8-minute-6-second response — translates to a 5-minute-36-second head start for bystanders, more than half the survival window cardiologists care about most.

"This is about using every tool we have to keep people safe," said JCCPD Police Chief Mark Jamison. James City County Fire Chief Ryan Ashe added: "In cardiac arrest, minutes matter. If we can get an AED to a bystander faster, that's a win." VCU emergency medicine professor Joseph Ornato, who co-leads the research as part of the multi-site RESTORe-CARE study, said the results "show how drones could give rural communities a critical head start in saving lives."

James City County joins Forsyth County, North Carolina as the only two US localities flying AEDs to live cardiac arrest calls under the RESTORe-CARE study, which Duke and VCU researchers plan to expand to additional counties as data comes in.


What This Means for Drone Pilots

Drone-as-first-responder (DFR) programs are one of the fastest-growing categories of public safety drone use in the US, alongside programs from Skydio, Brinc, and other dock-based operators serving police and fire departments. James City County's FAA waivers illustrate a recurring bottleneck for these programs: routine, dispatch-triggered flights beyond the operator's direct line of sight still require case-by-case approval rather than blanket authorization, a gap the FAA's pending BVLOS (Beyond Visual Line of Sight) rulemaking — explained in PickDrones' BVLOS guide — is meant to close.

For commercial and public-safety drone operators, the results here add concrete numbers to the case for AED-carrying DFR contracts, particularly in rural and suburban counties where fire response times routinely exceed 8 minutes. Expect more county sheriffs' offices and fire districts to request similar waivers once RESTORe-CARE publishes full outcome data.
